Professional Development Instructor Application ^{_{( 96kB)}}
The application must also include a photocopy of your teaching license or photocopies of your graduate and undergraduate transcripts.

I-9 Form ^{_{( 380kB)}}
All instructors must complete an I-9, which is a verification of employment eligibility.

W-4
All instructors must file a federal ^{_{( 164kB)}} and state ^{_{( 312kB)}} W-4 form with Heartland so that the appropriate income taxes are taken out of the payroll check.
