OT & PT Services
A record kept by the therapist to provide
general information about each student regarding
child contacts, consultations, meetings, treatment
strategies, and plans. A grid is provided to record
the service date, type, and time. Therapists may
initiate a service record with the first contact
regarding a child or at the start of an IEP; and
begin a new service record at each annual review or
start of each new school year.
