Supervision of Occupational
Therapy Service Providers
OT & PT Services
Supervision
- COTA
- Practice only under supervision of
Iowa-licensed occupational therapist.
- Require a minimum of 4 hours per month
on-site and in-site supervision by OT.
- Must designate on a board-approved form
the name of the supervising OT. Must notify
Board in writing within 7 days of when change
takes place.
- Supervision
plan and documentation
of supervision shall be kept by COTA and
OT.
- OT-Limited
Permit
- Practice only under supervision of
Iowa-licensed occupational therapist.
- Require a minimum of 2 hours per week
supervision by OT during one-to-one
supervision.
- Must designate on a board-approved form
the name of the supervising OT. Must notify
Board in writing within 7 days of
changes.
- Supervision
plan and documentation
of supervision shall be kept by limited
permit holder and OT.
- OT & OTA
Students
- OT student supervisor must be an
occupational therapist with at least one year
of experience in the setting.
- OTA student supervisor must be an
occupational therapy assistant or
occupational therapist. One year of
experience in the setting is required. In
Iowa, if the student were supervised by a
COTA, the supervising OT would also be
responsible for the student.
Service Delivery
- COTA
- OT shall ensure that COTA is assigned
only duties and responsibilities which they
have been specifically trained and qualified
to perform.
- OT shall:
- Evaluate each client prior to COTA
intervention.
- Write treatment plan outlining
elements delegated to COTA.
- Monitor client progress.
- Evaluate treatment plan and determine
treatment termination.
- Best practice would indicate that the
COTA collaboratively assist the OT in these
tasks, when appropriate.
- Licensure law states that time evaluating
a client does not count towards supervision
of COTA.
- OT-Limited
Permit
- OT shall ensure that limited permit
holder is assigned only duties and
responsibilities which they have been
specifically trained and qualified to
perform.
- Under the supervision of the OT, a
limited permit holder shall perform duties of
occupational therapist, except providing
supervision to COTA.
- OT & OTA
Students
- The OT is responsible for:
- Evaluating the client.
- Developing a treatment plan,
delegating specific treatment
responsibilities in writing.
- Monitoring client's progress, changing
treatment plan, and determining the
terminating of treatment.
Documentation of Services
- COTA
- COTA shall:
- Document every student related contact
(i.e., phone calls, meetings, interventions)
on service record.
- Show documentation to supervising OT;
place documentation (i.e., contact sheet) in
student file in OT area at least
annually.
- Have all official documentation (i.e.,
reports, letters) cosigned.
- OT-Limited
Permit
- OT limited permit holder shall:
- Follow policies for documentation at
agency.
- Share documentation with supervising
OT.
- Have all official documentation cosigned
by OT.
Other
- COTA
- COTA services under supervision of an OT
are considered occupational therapy.
Heartland lists the amount and frequency of
OT services on the IEP and IFSP to reflect
OT, and COTA when appropriate, personnel. The
person (s) responsible for carrying out the
service are listed on the IEP and IFSP.
- OT-Limited
Permit
- Limited permit is valid until date on
which results of exam have been made public.
Limited permit not renewed if applicant
failed examination.
- OT & OTA
Students
- Care rendered by unlicensed person shall
not be held out as, and shall not be charged
as, occupational therapy unless direct
on-sight supervision provided by OT.
