Transition Assessment Tools

Hints for gathering yearly data

  • Determine check points when data needs to be collected
  • Target certain weeks during the first and second semester to gather data
  • Gather data during parent-teacher conferences
  • Ask counselors what they are doing for all students for vocational assessments and utilize that data
  • Find out if any general education courses have any vocational assessments or activities such as interest inventories or preparing resumes imbedded in the curriculum and utilize that information
  • Teachers do scheduled weekly transitional activities

  • Have someone read your IEP for clarification
Organizational Tools-Suggestions for teachers
  • Keep a hard copy of assessments including transition questionnaires, interviews, and discussions--make sure the information is dated to show that the transition data is on-going and gathered yearly
